Transition Information
To further encourage and support our students and families, we motivate our parents and students to complete the Level 1 Vocational Assessment. This provides resources for parents to learn about, attend, and gain OPWDD-eligibility for their young adults through the Front Door Information Sessions and access Care Coordinator Agencies. Lastly, the assessment affords us the opportunity to share Article 17-A Guardianship resources as part of Post-Secondary Readiness and Transition planning during IEP Annual Review Meetings.
Additionally, our annual Internship Fair provides opportunities for students and families to learn about the specific vocational programs offered at P721K. Throughout the year parents are offered many opportunities to participate in school-based and district-based workshops to help prepare their young adults for transition and post-secondary outcomes. During our annual Transition Fair, community-based agencies meet with our families to provide information on available services, programs, and referrals needed as our learner's transition toward commencement.
